Radiant Wellness Club (RWC) Recurring Dues Membership Agreement
Our Radiant Wellness Club (RWC) membership program is designed to enhance your well-being with exclusive benefits provided by Zen Dermatology. Please carefully review the following Terms of Membership, as they outline your rights, obligations, and the conditions associated with your membership.
Membership and Recurring Payments
By selecting Zen Dermatology’s RWC membership plan, you consent to these Terms and authorize Zen Dermatology to charge your credit card in the amount of $99 each month. The $99 fee will be payment for your membership benefits, including 1 intramuscular injection (IM) per week and your choice of either 1 red light therapy session or 1 infrared sauna session per week.
The recurring charge on your card will be a monthly, recurring transaction. Please note that you must provide a 30-day cancellation notice if you wish to terminate your membership. Unused benefits do not rollover.
Consent to Recurring Payments
By accepting these recurring payment terms, you acknowledge that you consent to receive electronic copies of your payment authorization for recurring monthly payments to Zen Dermatology. This consent is exclusive to electronic form, and you cannot withdraw it once accepted. However, you retain the ability to cancel your recurring payment authorization in accordance with the terms outlined below.
Changes to Price and Membership Plans
Zen Dermatology reserves the right to adjust pricing or change the membership plan at its discretion. Changes will take effect following notice to you.
Updating Your Payment Methods
You can update your payment method by calling our clinic or by visiting in person to fill out a new form. Authorization to charge the updated payment method(s) will continue after any update.
Continued Obligation
If a payment is unsuccessful, and you do not cancel your account, access to Zen Dermatology membership benefits may be suspended until a valid payment method is successfully charged. Certain fees may be charged by the payment method issuer.
Canceling Your Membership
Your RWC membership is month to month with no contractual obligation and will automatically renew until terminated. To cancel, contact Zen Dermatology by email at info@zendermatology, deliver written notice to the clinic, or send written notice via certified mail. Cancellation requests must be received at least thirty business days before the next payment date. Early termination will result in a $99 fee.
No Refunds or Substitutions
Payments are nonrefundable, and no refunds or credits are provided for unused or partially used benefits. If you choose not to utilize your benefits, they will not rollover, and no credit will be issued.
Membership Benefits and Payment Method Authorization
I understand that the benefits available through my RWC membership are non-transferable and do not carry over from week to week. The benefits include 1 intramuscular injection (IM) per week and your choice of either 1 red light therapy booth session or 1 infrared sauna session per week as stated above.
By signing below, I affirm that I am of legal age to enter into this agreement, and that I have read, fully understand, accept, and are bound by the Terms of Membership for the Radiant Wellness Club at Zen Dermatology.
I hereby authorize Zen Dermatology to charge my account on file $99 each month.
